The form provides a clear structure for students to select their preferred payment method, including options for credit card transactions, payroll deductions for Meditech employees, and instalment plans through Debit Success. Additionally, it outlines important terms and conditions related to cancellations, refunds, and administrative fees.

Design Objectives

The primary design objective was to create a professional, easy-to-navigate document that aligned with Meditech Training College’s branding and communicated critical financial and policy information clearly. The design needed to present detailed content in a way that was approachable for students, while also serving administrative needs.
